Referral and Admission Procedures:   Call and discuss the referral with the Admissions        Coordinator: Erin Walker, ewalkerbyr@aol.com        or the Clinical Case Manager: Sharon Perez,        shperezbyr@aol.com Fax or e-mail a psychological evaluation that is not more than 12 months old and a completed common application to: 254-829-1782 The Brookhaven Treatment Team will evaluate        the referral information and consult with you,        typically, within 2 hours If accepted, arrangements for transport will be        discussed and an admission date established. Other necessary documents should be brought        with the resident upon admission   Fees:   Fees are based upon state approved        reimbursement rates for LOC V and VI. Brookhaven is title IV-E approved. The average length of stay is 6-9 months.
