Referral and Admission Procedures:

	Call and discuss the referral with the Treatment Director
	Fax or mail a psychological evaluation that is not more than 6 months old
	Complete and send a "Common Application".
	The Brookhaven Treatment Team will evaluate the referral information and

           consult with you, typically, within 24 hours

If accepted, arrangements for transport will be discussed and an admission date

           established.

Other necessary documents should be brought with the resident upon admission

Fees:

Fees are based upon state approved

           reimbursement rates for LOC V and VI.

	Brookhaven is title IV-E approved.
	The average length of stay is 6-9 months.
